St. Paul's was the first organization to bring St. Paul's PACE (Program of All- inclusive Care for the Elderly) to San Diego in 2008. The PACE program is an all-inclusive health care plan that provides low-income seniors with wraparound medical services, transportation, mental health care, home care, nutrition, physical therapy, and more. Together, these services allow seniors to thrive while living safely in their own homes. St. Paul's PACE has locations in San Diego, Chula Vista, El Cajon and Encinitas; they continue to innovate and expand in order to ensure all San Diego seniors have access to the care they need. 111 Elm Street San Diego, CA 92101 1.833.PACENOW stpaulspace.org • St. Paul's PACE currently serves more than 1,100 low-income seniors in San Diego. • St. Paul's PACE has housed more than 300 formerly homeless seniors in supportive housing communities throughout the county. • St. Paul's PACE is the first PACE program in San Diego to launch a Clinic-on-Wheels, bringing health care directly to its patients' homes. • Want to donate?
